- Confirmation upon receiving a referral, including details of initial appointment booking.
- To work collaboratively with all providers involved in the management of a client’s health plan.
- Timely written reporting including:
- A comprehensive written report following the initial appointment
- Progress updates as requested or required following the achievement of treatment milestones, or updates or changes to the management of a client’s plan.
- A final report on the completion of a management plan (Workers’ Compensation / TMP) or 3 weeks prior to a plan review (NDIS).
- Attendance at plan reviews including:
- Workers’ Compensation Case Conferences (NDT meetings) as requested or as required.
- NDIS plan reviews (with case coordinators or LACs) as requested or as required.
- The delivery of friendly, patient centered and evidence based exercise programs focused on achieving the plan goals whilst tailored to the clients’ needs.
